Pain in chest, shoulder and back

Since 3 days I'm having severe pain in my right side chest along with shoulder and back. When I breathe, cough and sneeze I feel a very sharp pain. Also last night I felt a burning sensation in my heart and throat. Also very difficult when swallowing food.
