Need medicine names

Hi, I am unable to read out medicine names... Can any kind doctor help me with the prescribed medicine details so dat I can order these from any shop. I spoke with my local shop but dey r unable to read out the prescription. My doctor who prescribed me medicine is currently out of station and is not picking up calls. I will be really grateful and thankful if any kind hearted doctor can help me with the name of medicines with potency and any instructions to prepare that  prescribed in attachment ...So that I can get it from local shop. Plz help me...
