Need help please

Hello Doctor, For over 20 years, despite psychiatric treatment, I have had episodes of burning in my head, palms, feet, and near my ears, along with a urge to pass motion. My hands and feet may suddenly become cold, followed by intense fear/panic and an urge to escape. These episodes are triggered by minor stress, sad memories, or sometimes occur without any trigger. I also feel detached from reality, as if I am living in a dream and not fully conscious. I feel persistently depressed, negative, and unmotivated. I constantly worry about losing my parents and often wonder how I will cope with life. I have taken Ativan, Tryptomer, Nexito, Venlor XR, Olaset, Prexep, and Serta, but my symptoms remain. I feel better only when sedated, which is not practical for work. Could these symptoms be due to anxiety/depression alone, medication effects, or another medical problem (heart, diabetes, thyroid, vitamin deficiency, liver, kidney, or nerve disorder)? What should I do?
