Hello, you may not need surgery if this is an acute tear. A recent one. However, it definitely needs immobilization for a few weeks. May be 3 to 4 weeks. Surgery is usually required for those who have chronic pain and instability from the ligament tear. This happens because the ligament had failed to heal. There are a few exceptions which cannot be addressed in detail here. These mainly involve injuries to associated structures in and around the ankle joint.
