One thing that needs to be addressed is why there are degenerative changes at 27yrs. You may need further workup of blood to rule out arthritis. Do consult a Physiotherapist.

Can you visit a physiotherapist who is a shoulder specialist? Proper rehab can get you back in shape. You need clinical evaluation.

Acc to ur MRI, there r changes in ur muscle tendon...u shld try regular physiotherapy sessions for somedays so that the inflammation in n around that area can reduce, rest the arm using a sling , ice formentation..when the muscle is allowed to rest n recoup we can start strengthening it .this will take a few days as muscle injury will take more than 6 wks...if u find no improvement then we need to investigate or consult a shoulder specialist
