Facial and throat heaviness during periods is commonly due to hormonal fluid retention (PMS-related). Cycles and bleeding are otherwise normal, so it’s usually not serious.

The facial and throat heaviness or swelling you notice around your periods is most commonly due to hormonal fluid retention that occurs in the premenstrual and menstrual phase. Changes in estrogen and progesterone levels can cause the body to retain salt and water, leading to puffiness of the face, eyelids, and a sensation of throat or neck heaviness. This is a well-recognized premenstrual symptom and is usually benign, especially since your cycles are regular and bleeding is normal. Other contributing factors can include high salt intake, reduced physical activity, poor sleep posture, dehydration, or stress. The fact that the swelling is more noticeable during sleep and settles otherwise also supports fluid redistribution rather than a serious condition. In most cases, a routine gynecological consultation is sufficient. The gynecologist may advise basic tests if needed, such as thyroid function tests, hemoglobin, and electrolytes, to rule out common contributors like hypothyroidism or anemia. Extensive testing is usually not required unless the swelling is persistent throughout the month, worsening, or associated with symptoms like breathlessness, persistent throat tightness, weight gain, or swelling of legs. General measures such as reducing salt intake before periods, drinking adequate water, regular physical activity, and proper sleep posture can significantly reduce these symptoms. If needed, your gynecologist may suggest mild supportive treatment. Good luck!
