Demographics: 29M, Indian | 180 cm, 76 kg | Ex-smoker (5 yrs) | No chronic conditions
Current Meds: Azithromycin and cefixime (recently prescribed)
History of Present Illness:
Symptoms began in August following an acute diarrheal episode:
* Cyclic Fever: Low-grade fevers spiking primarily at night (~100°F rectal).
* Heat Intolerance: Rapid overheating and severe "burning body" sensation with sun exposure or mild exertion.
* Fatigue & Malaise: Persistent, profound exhaustion since onset.
* Sleep & Endocrine: Severe insomnia (unable to sleep until morning); sudden, steep decline in libido.
Investigations to Date:
* CBC: Normal
* LFT: Normal
* Widal Test: Inconclusive(1:80)but treated by doctor as typhoid .
Normal neck ultrasound for persistent pain in neck stiffness feeling .
* Chest X-ray: attached . No clear explanation.

X-ray chest and Blood report look fine..
Rest of your Blood investigations are within Normal range
Advice = keep continue taking
TAB AZITHROMYCIN 500 MG for 10 days and
TAB CEFIXIME 200 MG TWO TIMES for 5 days
TAB PARACETAMOL 500 MG THREE TIMES if fever persist.
Plenty of fluids
ORS packet dissolve in 1 litre of water take it few times in a day
Adequate Rest
Maintain your sleep cycle

Persistent evening fever with fatigue, heat intolerance, insomnia and reduced libido needs a fresh physician review; these symptoms should not be labelled typhoid from a single Widal result alone. A Widal titre of 1:80 is not diagnostic by itself.
Record the temperature with a reliable thermometer and carry your CBC/LFT reports, chest X-ray and all antibiotic details. Do not extend or repeat antibiotics without examination.
You may need a focused examination and targeted tests based on findings, including review of thyroid and other causes of prolonged fever.
