As per the what you have written here you do have 2 levels of lumbar disc bulges at L4-5 level with no nerve root compressing.L5-S1 level disc with disc bulge compressing Right s1 nerve root . MRI is very sensitive study ,it will show slightest changes.It all depends on your symptoms and  how disabling it is for you. You have not mentioned about it. Morever there's slight narrowing of spinal canal .

You must visit an orthopaedician preferably specializing in spinal problems at the earliest,since you complain of severe back pain.

This is a case of Orthopaedics particularly spine care. But as L4-L5 doesn't have any neurological compromise i don't think a surgery is needed however in case of second injury there might be something because of the tear..  but still its mild. So i need history of your complain before I can comment on the in depth issues
