1.If you are gym going guy then possibly issue related which you mentioned may be PFPS(Patello-femoral syndrome) which is dysfunction of knee joint due to over strain or overuse. It can be corrected by Hot pack, Joint mulligan mobilisation by physiotherapt mainly. other electrotherapy modalities if required that will depend on assesment by physiotherapist.
2. For low back pain,if you are leaning forward that means yours hams are tight & iliopsoas-abdominial muscles are weak. You have to strenghthen your weak muscles & stretching of tight muscles.
3.Start with core exercises(strengthening & stability).
4.Avoid jerks & heavy lifting and use hot fomentation on your back for 15 min.
