3.9 mm calculus in the kidney may  remain asymptomatic for years together.only medical treatment is advisable to such a tiny stone .removal too is quite difficult as it is usually very difficult to seek such a tiny calculus inside kidney which is a solid organ. RIRS is one modality to be suggested if removal is tried for administrative reason though medical recommendation is of medical treatment alone.
