Itchy scalp and hair falling out

I'm having itchy scalp and little dandruff also I can see when I use the scalp massager, my hair is just falling out and thinning on the scalp. I had really thick hair but now I touch it and it's falling out. Please suggest medicine or care. I've attached photos. My tsh was a bit high due to stress.
