Irregular periods , weight gain , pregna

I'm diagnosed by PCOD since 2 years but from past 6 months my periods are not regular and continuously spotting for more than a month. I'm getting periods within just 15 days. I'm not able to conceive and recently before 2 days , i found a flesh like structure in my period. What can it be? I'm really worried.
