Intermittent Fever

My uncle had 101+ (<=102) fever for a  week, recurring daily within gap of 6-8 hrs. Anticipating viral infection & since family doctor was out of station, we only used Paracetamol 650 when fever crossed 101. Conducted CBC, Urine Test & Culture, Typhoid, Typhus, Dengue, Malaria, CRP, PCT, etc. on another doctor's advice; all test results were good, except CRP and ESR (59 & 45 respectively). Family doctor returned on 8th day of fever & started Cefuroxime 500 twice daily - 7 days. After starting Cefuroxime, fever frequency reduced (1-2 times a day) and peak temps. fell below 101. CRP fell to 42. From 4th day of Cefuroxime dose, doctor started Doxycycline SL 100 twice daily - 7 days. After starting Doxycycline, for last 3 days body temp. is stable (97.8-99.4). Temp. crossed 100 once-twice but reduced on its own by slight sweating. My uncle takes high BP meds last 15 yrs and started sugar meds recently (borderline diabetic). Is fever improving? He looks fine & well. What could be the issue?

Hey there. It's a typical medicine case. You've provided a great history by the way. I see that the basic investigations are negative. Not knowing what's wrong is worse than knowing whats wrong. Next step should be doing advanced investigations like cultures, bone marrow biopsy etc. But for that you need to visit a hospital

A repeat clinical examination, and CBC should be done. If no cause is found he may have to go for blood culture
