Infection in Foot thumb

This is some kind of infection/allergy in my foot thumb.I had ingrown nail problem some years back which comes and goes in some time.I used to cut the nail from side on my own because I had a surgery for the same earlier but it recurred.Now from some months I have these red spots on skin which get more intense on pressing.I am getting treated for the same since last 2 months.Nothing seems working .I have taken Teczine,Halox F , Bularid L,Azifast but still it looks like same.From the outside the surface is plain/flat(no bumps or lumps)with no pain .It seems an internal infection with clear surface.What kind of infection is this ,how it occured(can this be occurred due to cutting the nail on my own) and how serious ?What can be the alternative treatment ?

It seems to be dermatitis along with paronychia. More photos are needed for accurate diagnosis so that treatment can be given to you accordingly.
