Dear Sir/Madam,
I am suffering from abdomen pain and yesterday i feel chest pain also. I consulted with doctor but i am not getting below terms can you please help me on this.
O/E
CUS - S,S2T
P/A - tender epigastrium

These are medical code terms:
O/E= on examination
CVS: (not cus)= cardio vascular system that is heart and vascular system
S1, S2 = heart sounds
P/A= per abdomen ( stomach)
tender epigastrium= pain + in the upper portion of stomach
