A PATIENT WITH COMMUNIATED FRACTURE HUMERUS , radial nerve palasy too AND intertrechontic fracture(already healed on its own ) 73 year old has ischemic cadiomypthy ejection fraction 40 % since when operated for first time govt hospital went to ventilater system for 2 days . how one should go about it for treating hand ? govt doctor afraid to oprte

If radial nerve palsy from beginning- it might recover. Life is more important than radial nerve. If not recovering then wrist drop splint. If you would like to do surgery - then do under block.

Conservative treatment for the #shaft of humerus considering his age and comorbidity.  Observe for radial nerve recovery. If no recovery in radial nerve patient can undergo tendon transfer under supraclavicular block. Early tendon transfer is also an option.
Next Steps
closed reduction and plaster for #soh if still early. or use a functional brace if already late.

wrist drop splint. EF 40% is not gr8. surgery might be life threatening
