HMD dust mite & cockroach allergy

2026-04-10 10:00:56
I am a 23-year-old male with a persistent dry cough and redness with cobblestone appearance at the back of my throat. My chest X-rays 3 times and Pulmonary Function Tests 2 times are normal, with recent values showing FVC 92% and FEV1 95%. CRP is negative, and I have no issues with eating or sleeping. My IgE level is high 682 and Recent allergy testing shows strong reactions to dust mites and cockroaches. A CT scan of my sinuses (March 16, 2026) shows a deviated nasal septum to the right, concha bullosa on the left, and mild sinus swelling, but normal drainage pathways. Its been 2 month 10 days to this problem but i got nasal spray after 1.5 month now cough is improved & its occasional Could this be allergic rhinosinusitis with post-nasal drip (UACS), and is nasal spray treatment & antihistamine is enough should I consider or HDM T Sensimune SLIT immunotherapy?

Based on your high IgE levels (682) and positive allergy tests for dust mites and cockroaches, your symptoms of persistent dry cough and 'cobblestone appearance' in the throat are classic indicators of Allergic Rhinosinusitis with Post-Nasal Drip (UACS). Since your PFT and Chest X-rays are normal, it rules out asthma or lower respiratory issues. Your CT scan shows a Deviated Nasal Septum (DNS) and sinus swelling, which often aggravates these symptoms. While nasal sprays and antihistamines provide symptomatic relief,
Next Steps
Sublingual Immunotherapy (SLIT) is a good long-term option to treat the root cause by desensitizing your immune system to specific allergens."
Health Tips
To reduce the allergen load at home, use dust-mite-proof mattress covers and wash your bedding in hot water (60°C) weekly. Since cockroach allergy is also present, ensure strict pest control in the kitchen and damp areas. Regular saline nasal rinses (Neti pot or spray) can also help clear the post-nasal drip mechanically Immunotherapy (SLIT) is a long-term commitment, often taking 3-5 years for full effect, and should only be started under the supervision of an Allergist or ENT specialist. Do not discontinue your current antihistamines or nasal sprays abruptly, as 'rebound congestion' can occur. If you experience any sudden shortness of breath or wheezing, consult a doctor immediately"

It does look like allergic rhinosinusitis.  DNS is also playing a part. Take steam inhalation 2 to 3 times daily. Regular pranayam will also help you. You can use Avamys nasal spray, 2 puffs just at beginning of cold. If you get DNS treated, much of the problem will settle. It is just a few minutes surgery done as old procedure.
