High Urea,Creatinine ,Uric Acid in blood

As per reports of my Liver & Kidney Panel,Serum (Lalpathlabs) , following are found: Urea - 77mg/dl (Ideal Range:18-55mg/dl); Creatinine - 2.23 (<1.20); Uric Acid - 8.40 (Ideal Range 3.4-7.0). Also I'm diabetes & hypertension patient.& Also having pain for past 1 month in one leg joint. Kindly suggest some remedial measures & whom I should consult further. Thanks in Advance.I'm new to this platform so kindly forgive me if anything I have missed.
