High triglycerides and low hemoglobin

My father aged 59 years, a CABG surgery operated patient, creatinine at baseline 2.5, uric acid, diabetic patient Hba1c at 8.2. Heart and kidney has been well managed by medicines. He has been further taking erythropoietin injection twice a month, nephro doctor has suggested for low hemoglobin to increase to 4 times while checking the new report His diabetes is badly controlled due to his erratic diet. But he was further diagnosed with high triglycerides. Kindly do help us to lower his triglycerides level and increase his hemoglobin level.

His hb is low because of kidney issue pls correct that and consult online for medications
