My nephew's head is hot all the time. And he is always feels warm despite the cold weather. He is very active but refuses to eat much.

Just document the temperature if it is below 100 °F then no need to worry.... For other compliant Give him frequent feeds.... What is weight and height of your child?

Head,palms and soles of feet are warmer than rest of the body in small children. Measure the temperature,if it's below 100,relax.
