Diltiazem can help esophageal motility, but it WILL lower blood pressure in most people. Since your BP is already 100/70, taking diltiazem may drop it further and cause dizziness, fatigue, or fainting. Safer approach: Diltiazem is NOT recommended at your current BP unless your gastroenterologist and cardiologist agree and monitor you. Very low doses (30 mg) still reduce BP in many patients. Better options your doctor may consider instead: Peppermint oil before meals (reduces esophageal spasm without lowering BP) Buspirone 5–10 mg (improves esophageal motility and LES relaxation; minimal BP effect) Low-dose nitrates only if supervised (also lower BP so usually avoided in low BP patients) Important: If food stays in your esophagus for hours and comes back undigested, you need evaluation for: Achalasia variant Esophagitis Stricture Severe motility disorder You may need HRM (manometry), EGD, or barium swallow depending on what’s done already. If you tell me exact prokinetics you tried, I can guide which alternatives are safer with low BP.

I am sure this must be distressing causing a hindrance in your day to day activities.I shall try to the best of my abilities to consult you Oesophageal dysmotility can be due to a variety of causes such as neural, hormonal, muscular and many more Yes Calcium channel blockers can affect both BP but works for esophageal dysmotility Therefore it will require extensive workup (history, investigations) to diagnose the cause of motility problem followed by curating  a treatment plan preferably using a single drug which does not affect BP or to make a system coupled with dietary and lifestyle changes
