Fracture shaft of humerus underwent closed intramedullary interlocking nail yesterday.today after 24 hrs after surgery, she is unable to extend the wrist.surgeon gave 2 options ,radial nerve might be entrapped in between fracture fragments or it might be due to pressure...shall we have to wait for the wrist movement or allow them to again open posteriorly to directly see the nerve?

Million dollar question is if she was extending the wrist pre surgery. If yes then definitely exploration urgently. Can wait for a day or 2 to see if any hint of recovery is there fasciculations etc. If she was unable to extend even prior to surgery, then you need to wait as usual for nerve recovery 6-9 months.

Pressure (compartment syndrome) building around nerve or nerve getting entrapped... as told by surgeon. both are medical emergencies... These should be clinically evaluated and explored at the earliest. There is no wait and watch for this.

U need to wait a minimum of 3- 6 weeks to see any improvement in the wrist n fingers extension. Do continue passive exercises and cock up splinting as advised by ur surgeon
