You will have to take a consultation since post thyroidectomy surgery the recurrent laryngeal nerve has to be assessed by doing a laryngeal endoscopic examination. If the cords are moving well , decannulation trial can be taken and tracheostomy tube can be removed if there are no adducted cords or any paresis present .

The recovery time and when the tracheostomy tube can be safely removed after a thyroidectomy (especially total thyroidectomy) varies depending on the reason it was placed and the patient's individual progress. Tracheostomy is not routine after thyroidectomy it's usually done only in specific cases, such as: Significant tracheal softening/weakness (tracheomalacia) from a large or long-standing goiter compressing the trachea. Airway swelling, bilateral vocal cord issues, or other complications affecting breathing. At 3 weeks post-surgery with the tube still in place, this is longer than average for many thyroidectomy-related cases, but it's not uncommon if there was tracheomalacia or other issues.
