Keeping yourself hungry for 18 hours can certainly cause hyper acidity, nausea, vomiting, headache, bodyache, fatigue, irritability..and more
Long term effects may cause peptic ulcers, mouth ulcers, pain while swallowing, bad breath.. the list goes on
Fasting once a week is not necessary and neither is keeping yourself hungry for 18 hrs.
