Excessively stretched foreskin

I have excessively stretched foreskin due to using my foreskin to collect the semen during masturbation by stretching it to cover the erect penis at the time of orgasm and not have the semen spread everywhere. Doing this over the years and frequently has stretched my foreskin a lot and is now causing problems either when masturbation or intercourse. Can the excess foreskin be removed or shrunk. Can this be done using medication or will it require surgery.
