Evaluation for Severe Weakness

Doctor, I have been experiencing severe weakness for a long time. Even routine household work feels difficult, and I get tired very quickly. I also have muscle pain, leg cramps, excessive sleepiness, constipation, and significant hair fall. My recent blood tests showed: - Vitamin D: 19.26 ng/mL (deficient) - Vitamin B12: 253 pg/mL (low-normal) - HbA1c: 4.9% (normal) - Thyroid tests: Normal - Hemoglobin: Normal - SGPT (ALT): Mildly elevated - LDL cholesterol: Slightly elevated Could my severe weakness be due to Vitamin D deficiency alone, or should I be evaluated for other causes such as low ferritin, Vitamin B12 deficiency, fatty liver, or any muscle-related condition? Please advise if I need any additional tests or changes in treatment.

Take supplements for vit d. Get serum calcium, proteins,  several hepatitis, also tested. Get  abdominal ultrasound done and share all these results. For slightly high ldl cholesterol,  avoid fatty meals, do regular physical exercise,  rest adequately.  Repeat test after 6 weeks.

Your weakness is likely multifactorial. Vitamin D at 19.26 is clearly deficient and alone can cause muscle pain, cramps, fatigue, and hair fall. B12 at 253 is low-normal — functional B12 deficiency (causing fatigue and muscle symptoms) can occur even in this range. The mildly elevated SGPT may point to early fatty liver, which contributes to metabolic fatigue. Importantly, ferritin (iron stores) can be depleted even when hemoglobin is normal, causing significant weakness — this hasn't been tested yet and is a key missing piece.
Next Steps
