My first-trimester screening shows an elevated free β-hCG of 3.16 MoM, but the combined screening result for Trisomy 21 is 1:8625 and is reported as low risk. The NT scan was also reassuring, with NT 0.7 mm and the nasal bone visualized. What would doctors generally recommend next in this situation—only a detailed fetal-medicine scan, or would NIPT also be advised? Could this variation in the double-marker test cause any problems for the baby after delivery? Is there any increased risk of health or developmental problems after birth because of the elevated β-hCG, even though the overall screening result is low risk?

An overall combined risk of 1:8625 with a reassuring NT and a visible nasal bone is a reassuring first-trimester screen, even when free beta hCG runs high. An isolated raised beta hCG here does not by itself predict a problem for the baby.
Next Steps
The routine anomaly scan around 18 to 20 weeks is the usual next step; NIPT is optional for extra reassurance.
