Keloids at shoulder are usually treated by injections or cryotherapy or both. A newer treatment is intralesional radiofrequency or cryotherapy
Next Steps
please consult a dermatologist in person for best advice

If you are prone to develop keloids, it is usually not advisable to undergo surgery as it can cause worsening of the situation
Next Steps
visit a dermatologist to discuss about decreasing its size and improving its texture. you can go ahead with medicated local injections or laser treatment.
