Your picture fits functional dyspepsia with bloating and IBS-type bowel habit, sitting on grade 1-2 fatty
liver. Celiac is ruled out, so nothing sinister here. The real gap is that your
B12 and iron did not rise on oral supplements - that usually means poor absorption or too low a dose, not a new disease. B12 155 and iron 43 with hair fall need proper correction, injections for B12 and if
ferritin is low, an iron protocol with vitamin C. The antidepressant was likely given in a low dose as a gut nerve modulator, not for depression.
Next Steps
Get
ferritin,
TSH,
HbA1c and a repeat CBC done. Continue the
B12 injections as advised, keep the PPI short-term only, and take the psyllium at bedtime with plenty of water. Cut alcohol, fried food and late dinners, walk 40 minutes daily - fatty
liver and hair fall both improve with that. Share your reports with me on a Practo consult and I will fine-tune the doses and tell you how long to stay on each. Go to a hospital if you get black stools, vomiting blood, unintended weight loss or trouble swallowing.
