Diffuse disc bulge at L4-L5

IMPRESSION: Diffuse disc bulge at L4-L5 with left foraminal protrusion and posterior annular tear causing compression on the thecal sac with ligamentum flavum hypertrophy. Bilateral neural foramina are narrowed with compression on bilateral exiting nerve root (L>R). Diffuse disc bulge at L5-S1 with broad based protrusion and posterior annular tear causing compression on the thecal sac and bilateral traversing nerve roots in lateral recess with secondary canal narrowing AP canal diameter 9.4 mm. Bilateral neural foramina are narrowed with indentation on bilateral exiting nerve root. This is my MRI report . I wanted to know if surgery is required or it can be treated with physiotherapy and exercise

Surgery cannot be advised based on MRI scan alone Your main symptoms, duration examination and response to medical management will determine the need for surgery It can be generalised on based of a MRI report
