Difficulty passing tight stool

Hello Doctors, I had a c section delivery 5 months ago. On 9th day I saw blood in stool with burning sensation and small lump outside of anus. I guess I strain but little bit not much. Doctor said it's piles and fissure. Fissure healed completely and for piles also he said it will go in next 20 days. i dont have pain while passing stool now but small painless  lump is still thr .I was on stool sofner for 4 months. 10ml evict as more dosage was causing Diaherra. I have stopped stool softner and and on 4th day I had a difficulty passing stool. Stool was not hard. I drank hot water and tried after 2hrs again and then I had a motion. But first attempt irritated my anus. It is difficult to pass slight tight stool which I used to pass very easily earlier .if stool is puffy and mushy then only am able to pass easily. I am breastfeeding mother with lots of fiber in my diet and water intake around 5 to 6 ltr. please help why this is happening.
