Diarrhea for more than a week

My child was passing frequent foamy motion for more than a week. She poops 3 to 4 times. We gave her the medication recommended by a pediatrician followed by antibiotics. Still she has the same problem. Not sure if she is having diarrhea or she has changed the pattern of excretion.

first of all look for any dehydration ?? like dry mouth, decreased urine output, lethargic condition
