Can anyone suggest if surgery is needed for the attached results? I had this tear back in 2022 July. Pain increases at night, while waking up at my right shoulder.

As it is an old mri You need to get clinical examination and if required then you have to repeat the mri

Is there any difference  in symptoms from the day of mri and today. Surgery is not needed, Consult orthopedic surgeon for the re assessment of shoulder pain.
