Assuming your father weight around 70 yrs S Creatinine of 4.5 indicates GFR of around 15 ml/min. The symptoms of loss of appetite can be explained to some extent by the kidney disease but he needs evaluation for other causes as well
Next Steps
Need to find out the cause of increase in S creatinine and to look for any reversibility. Urgent consult with Nephrologist is required

When both kidneys stop working to their full capacity , creatinine ( a waste product) rises in  blood This is what's happening with your father Now this can be temporary or permanent ( if lasts for more than 3 mths) Also the cause of decrease kidney function can be Diabetes Hypertension Kodney diseases Bilateral kidney stones Prostate
Next Steps
consult a nephrologist and/ or urologist they will order several  tests which will help doagnose  why this is happening and also start treatment.
