Constant dandruff

I have been facing dandruff problem with constant itching, irritation and flaking. Because of dandruff my hairs are also falling. I have tried many means to control these problems but till now i am struggling. So, kindly suggest me what can i do further to solve this problem. Thank you, Eagerly waiting for your kind response
