"While some discomfort and swelling are expected for 48–72 hours after a circumcision, persistent burning sensations and difficulty urinating on Day 4 indicate that the surgical site is likely experiencing significant inflammation or a developing secondary infection. The burning sensation is a clinical sign that the raw tissue is being irritated by urine or is hypersensitive due to the healing process. At this stage, symptomatic management needs to be balanced with a close physical inspection to rule out complications like meatal stenosis or localized cellulitis

Loose Clothing: "Keep him in very loose cotton clothing or allow him to remain without a diaper/underwear for a few hours daily to allow air circulation to the wound."
Hydration: "Ensure he is drinking plenty of water. Diluted urine is less acidic and causes significantly less burning when it passes over the surgical site."
Caution: "Do not try to forcefully pull back any remaining skin or remove surgical scabs, as this will cause bleeding and increase the burning sensation."
Red Flags: "Seek emergency medical care if he is unable to pass urine for more than 8 hours, develops a high fever, or if the swelling and redness begin to spread toward the scrotum or abdomen."
