Teeath ache due to cavity which is already filled before 4 yrs. NOW opened and hurt make severe pain.

Please visit your nearest dentist for Root canal treatment of the tooth in question followed by Capping.
Health Tips
Don't prolong the treatment by taking over the counter painkillers. Otherwise it may worsen Periapical infection.

It seems like there is pulp infection which will require rct. Visit a dentist asap and get the necessary treatment done.

It's is due to prolonged infection. U need not to worry. As u said. I expect a rc to be done,which would solve the problem.
