We have done the CT scan as per the report concussion we find the below are details. Can you please suggest the best option what next steps need to take care TECHNIQUE: 32 slice volume scan of Chest was done without IV contrast CONCLUSION • Trachea is centrally placed with normal lumen and wall. The mainstem bronchi are normal Posterior Basal consolidations in B/L. lower lobe. Diffuse patchy pneumonitis in B/L lung, predominantly in upper zone . Ascending, arch, descending aorta and its branches, main pulmonary artery and its major branches, SVC and its tributaries show normal caliber and position. ⚫ Cardiac configuration is normal. No obvious mediastinal or hilar lymphadenopathy. . Moderate B/L. pleural effusion. Esophagus appears normal Visualized bony parts appeared normal.

CT chest suggestive of parapneumonic effusion . Needs admission and both diagnostic and therapeutic pleural fluid aspiration.
Next Steps

Can be Viral Pneumonia ( Covid or any other viral) . Get RTPCR for Covid 19. Start the antibiotics therapy along with send Sputum G/S , KOH, Pyogenic C/S , Sputum cbnaat for TB. Send. Blood Culture also. If no diagnosis made , then aspirate pleural fluid and send for investigations.
