Breast pain and swelling near nipple

My wife is suffering from breast pain and lump like swelling near nipple. Little nipple retraction near swelling. She has done lot of household  work that week. Attached MRI and biopsy. Please check what is the issue and treatment. Already started on antibiotics. Thanks in advance.
