My son 4 years old he going lose motion with gas for daily 3-4 times from 5 days and stool come up with blood and mucus also seeing in stool but he normal and active  what should I do

As per you history which you gave it looks like dysentery. Start ORS and zinc Consult pediatrician for further evaluation and may be required antibiotic and probiotics.
