While your TMJ does show slight bony changes at the joint it won't be possible to diagnose anything further from just the scan and no clinical examination. Your dentist is right in suggesting you visit a specialist. Ideally a TMJ/ joint specialist is the right choice for you.
You have also not mentioned what treatment you have undergone for the Joint pain. Any muscular therapy, use of a night guard or occlusal splint and low frequency biomodulation for the pain. These are the normal treatment options for joint issues alongside muscle relaxants. Have any of the above been tried?
