An anal fissure is a breakage in anal lining epithelium. Lump in that area mostly corresponds to a pile. But an anal fissure in close proximity to a pile calls for more bleeding and pain and chances of a pile being infected. So it has to be definitely examined by any surgeon (not physician) and act accordingly before things worsen.
