Advice on how to reduce sugar & BP level

My father is a Diabetic patient with High BP problem. Recently, when he got his BP & Sugar levels tested, he was diagnosed with a BP of around 160 and sugar levels of around 365 which is way beyond the normal levels. Pls advise on how he could bring down sugar & BP to normal level.

Is he on any medications already If so then he may need optimisation of his medications . if not already than he need to initiate appropriate treatment to get things under control . Appropriate diet will also go a long way in getting things under control , both in terms of diabetes and BP , along with medications
