Wounds in leg

Hello doctor, my friend got wounds in his leg. It's been several years since this started. No medicine helped. Photos attached. Can you please give your suggestions? He told he once carried a dead body to graveyard without wearing slippers and from that day he got these wounds. Not sure if this is some allergy or some insect bite but its there for many years. Please help.

Your friend is very obese . Test for diabetis mellitus. Take tablet Augmentin 625 mg twice a day, tablet Hifinac-P twice a day for a week. Capsule Cobadex forte once daily. Dress the wound regularly. Keep the limb elevated at night. Reduce weight by 15-20 kg. If diabetis mellitus is detected start medication immediately.
