30M, 170 cm, 70 kg About 4 years ago, I developed recurrent bacterial UTIs (~10 episodes), treated with multiple antibiotics based on urine cultures showing different bacteria. My main symptom was burning during urination. During this period, I lost 7–8 kg of muscle mass and have never regained it. Later, a prostate massage followed by urine culture identified bacteria, and I was treated accordingly. Since then, urine tests and cultures have remained normal, but my symptoms never improved. Current symptoms: Burning during urination Burning sensation in the urinary/genital area Penis shrinkage/retraction sensation No morning erections for 4 years Persistent inability to regain lost muscle mass Sudden facial wrinkling (started ~1 year after infections) Bilateral knee pain (started ~2 years ago) Recent back pain Despite normal urine cultures for several years and seeing multiple doctors, these symptoms persist. Question: what can be reason? what tests need?
