Vomits milk after every feed

My son is 4 month old and he is on  formula feeding and i am also feeding him together at the same time...i am not getting what is the issue he vomits all the milk from mouth after every feed plz suggest do i need to consult with a doctor.is there any issue of feeding both kind of milk together means mine and formula.
