Very low sperm amount upon ejaculating

This is an embaressing one for me... I have recently starting having an issue in the bedroom depo. When I ejaculate practiclly nothing comes out, very very little semen. I am 40 I was on HRT for a while but not for 6months or so. What can I do to resolve this issue? Thank u
