High BP, insulin resistance and fatty
liver are suggestive of metaboly syndrome, are you obese or over weight. In case you are morbidly obese that is BMI >32.5 with all above conditions you may need evaluation for bariatric surgery. In case you are normal weight/over weight but not obese you still require evaluation for hypertension at an early age to rule out conditions such as renal artery stenosis, pheochromocytoma,etc. Sinus tachycardia may be a side effects of medications for high BP or it may be part of the disease itself like pheochromocytoma
