The image is firstly not great.... but going with what we have at hand, the bigger cavity is in the 2nd molar and a cavity of this magnitude causes the infection..... having said that the cavity in the 1st molar is also relatively deep enough to cause an infection.
This x ray must be correlated clinically to decide the plan of action.....prima facie looks that 2nd molar is more troublesome than the 1st.
