The X-ray is not clear, second molar definitely needs root canal treatment, first molar needs to be evaluated again clinically but preferably after the treatment of second molar is complete, and most importantly, extraction is not the treatment of choice for any of these two teeth.
The situation can definitely get worse over time. It would not be impractical to follow a conservative approach in which you get your second molar treated first and then wait for the symptoms to be evaluated in the first molar.
Please search for an endodontist and get the treatment done as soon as possible rather than spending time on gathering varied opinions on an online platform, because the damage is still progressive while you read this message.
